A 65 year old man presents to the Emergency Department with a 2-hour history of central, crushing chest pain, which he describes as radiating into his jaw. He feels nauseated and sweaty. PMH. Hypertension, hyperlipidaemia. DH. Ramipril 10 mg once daily, atorvastatin 20 mg once daily. He has not taken any medication for his chest pain before arrival.

On examination
He appears pale and clammy. Heart rate 96 beats/min, blood pressure 138/84 mmHg, oxygen saturation 97% on air.

Investigations
ECG reveals ST elevation in leads II, III and aVF. Bloods have not been taken at this stage.
